What are some potential issues for passports or government IDs?

There’s quite a few.  But here’s the top ten:
 
Specific finished photo size
Specific size of the head size from crown to chin
     (Sometimes, the head size has to be from top of hair to chin; not the above)
Head has to be the right height from the bottom
Foreign passports use metric, which confuses us metrically-challenged Americans
No “unnatural” expressions
No face or background shadows
No reflections in glasses
Correct background color
Correct brightness and contrast
Sometimes earrings are not allowed
Sometimes head coverings are not allowed
 
And of course there can be more….
